Men who smoke tend to hear about their lungs and their heart. The conversation rarely reaches the part of the body many of them would be most motivated by.

It is a circulation problem first

An erection is a plumbing event before it is anything else: blood has to arrive quickly and stay. That depends on arteries widening on demand, which depends on a healthy lining inside the vessel producing nitric oxide.

Smoking damages exactly that lining. The result is the same process that narrows the arteries of the heart, happening in vessels roughly a tenth the diameter. That size difference is the reason erectile difficulty often shows up years before any cardiac symptom — the small pipes silt up first.

This is worth restating plainly, because it is the part men miss: erectile difficulty in a smoker is frequently an early warning about the arteries in general, not an isolated bedroom problem. It deserves a conversation with a doctor for reasons that have nothing to do with sex.

What it does around the prostate

Two threads run through the research, and they deserve different levels of confidence.

Inflammation. Smoking raises systemic inflammatory markers, and chronic low-grade inflammation is one of the mechanisms most often proposed in prostate enlargement and in prostatitis. This is a plausible and well-supported connection at the level of mechanism.

Prostate cancer. Here the picture is more specific than "smoking causes cancer". The evidence linking smoking to getting prostate cancer has been inconsistent. What is better established is an association with worse outcomes in men who already have it — more aggressive disease, and higher rates of recurrence after treatment. Those are different claims, and it is worth keeping them apart.

Fertility, for the men it applies to

Smoking is associated with lower sperm counts, reduced motility and more DNA damage in sperm. For a man trying to conceive, this is one of the few factors entirely within his control, and one of the faster ones to respond.

The part worth hearing

Vascular function is not a one-way door. The lining of the blood vessels begins recovering within weeks of the last cigarette, circulation improves over months, and sperm parameters improve over roughly three months — the time it takes to produce a new cycle of sperm.

Men who have smoked for thirty years often assume the damage is banked and there is nothing to gain. For the specific things discussed here, that is not what the evidence shows.
