What is this? How to define BPH ?
  • Benign prostatic hyperplasia (BPH) is a non-cancerous enlargement of the prostate that restricts the flow of urine from the bladder.1
  • There are two growth periods in a man’s life when the prostate enlarges. The first occurs early in puberty, when the prostate doubles in size. At around age 25, the gland begins to grow again.  This second growth phase often results, years later, in BPH.2
  • In BPH, the prostate becomes enlarged to the point that it presses against the urethra like a clamp on a garden hose. The bladder wall becomes thicker and irritable.  Eventually, the bladder weakens and loses the ability to empty itself, so urine remains in the bladder.  This “clamping” of the urethra and partial emptying of the bladder causes many of the symptoms associated with BPH.2
  • While a definitive cause of BPH has not been identified (possible the enzyme Aromatase), factors that contribute to the condition include hormonal changes associated with aging and family history1, and also nutrition and lack of exercise
  • BPH can be diagnosed by several means depending on the patient’s symptoms, including a digital rectal exam, the Prostate Specific Antigen (PSA) blood test, a urine flow study or rectal ultrasound.3

Treatment Options

- Convencional medicine:
  • Drug Therapy:1 Useful for treating the symptoms of BPH, but patients must remain on the medication for the rest of their lives or symptoms will return.
    • Alpha blockers: These medications – including tamsulosin (Flomax®), terazosin (Hytrin®), doxazosin (Cardura®) and alfuzosin (Uroxatral®) – act by relaxing the smooth muscle cells of the prostate and bladder neck to improve urine flow and to reduce bladder outlet obstruction.
      • Adverse effects may include: dizziness, fatigue, postural hypotension (drop in blood pressure on standing), nasal congestion and abnormal ejaculation.
    • Anti-androgens. These medications – including finasteride (Proscar®) and dutasteride (Avodart®) – inhibit production of the hormone DHT, which is involved with prostate enlargement.
      • Adverse effects may include impotence, decreased libido and decreased ejaculate volume.
  • Nonsurgical Treatment: Research has proven that minimally invasive treatments are safe and effective.  May be appropriate for patients who don’t want to take daily medication for the rest of their lives, but do not want to expose themselves to the risks of invasive surgery.

Discounting Sexual Dysfunction in BPH In the survey of 1,275 urologists and primary-care physicians (PCPs), respondents indicated that they believed that approximately 25% of patients experienced sexual dysfunction, while MSAM-7, a major epidemiological study of nearly 14,000 men published in 2003, places the incidence at 50% -- twice as high as physician perception. Surprisingly, urologists, who were shown by the survey to treat BPH twice as frequently as PCPs, were more likely to underestimate the incidence of sexual dysfunction associated with BPH (19% of their patient population for urologists vs. 27% for PCPs) or BPH medication (19% for urologists vs. 24% for PCPs).

More than half of all men over age 60 have the condition -- and after age 80, men have an 80% chance of developing the condition. Left untreated, the symptoms may progress, leading to serious health problems including urinary tract infections, bladder and kidney damage, bladder stones, incontinence and acute urinary retention.

Lower urinary tract symptoms related to enlarged prostate are also strongly correlated to sexual problems, including Erectile Dysfunction (ED) and Ejaculatory Dysfunction (EjD). In fact, lower urinary tract symptoms are a risk factor for ED independent of other conditions associated with ED, such as diabetes, hypertension, cardiac disease and hyperlipidemia.

Men with severe urinary symptoms report 50% less sexual activity and 33% reduction in overall sexual satisfaction compared to men without severe lower urinary tract symptoms. "We've seen in our practices that the majority of men over age 50 consider an active sex life important," adds Dr. Seftel. "Since men with BPH symptoms are already at risk for compromised sexual function, it makes sense to consider sexual side effects when treating urinary symptoms of BPH."
